If you would like to enroll your child for the upcoming school year, fill out the application and submit to Amy Roberson either in person, or, scan the completed form to aroberson@pentwater.k12.mi.us 

Every child is required to have a health appraisal before entering the preschool classroom on the first day. These guidelines are requirements by the State of Michigan Department of Health & Human Services and every detail counts! Pentwater Preschool is required to have a Health Appraisal on file for each child to maintain full compliance. The health appraisal must be completed and signed by the doctor, but please note that some areas will need to be filled out by the parents. Incomplete forms will not be accepted and we will ask you to return to your doctor’s office.
